The E/TFHC’s mission

The mission of the Edinburgh/Trafalgar Family Health Centers, Inc., is to improve the health status of both its patients and the communities it serves by providing high quality, family-oriented, primary and preventive healthcare services, with a particular emphasis on helping the poor, the medically underserved and vulnerable residents. This mission will be accomplished by:

• evaluating the health and health-related needs of the community
  developing and coordinating resources that have an impact on the community’s health care
  
  
• providing comprehensive, primary health care services to targeted populations, while accepting all people regardless of ability to pay
  
  
• coordinating and case managing the continuum of care for the patient population
  
  
• helping patients overcome language, economic, cultural and geographical barriers to obtaining health care
  
  
• positively impacting the overall health of the community by:
  
  
  • promoting community awareness of the need for regular health care, plus
  • stressing prevention and health promotion; and
    
    
• creating an organization that demonstrates effective project management, high provider productivity, and economic self-sufficiency.
